Chronic Disease Management
Chronic disease is typically described as long duration and slow progression. Common chronic diseases include diabetes, emphysema, asthma, arthritis, heart disease, obesity, osteoporosis and hypertension.
Our approach is to raise awareness, provide preventative care and advice, and ultimately help support sufferers of chronic disease to proactively manage their conditions. We currently provide diabetes management and treatment, Electrocardiogram (ECG), heart tests, arthritis treatment, asthma treatment and chronic kidney disease management.
Your doctor will create a chronic disease management plan for you that meets the needs of your lifestyle to help you treat, better manage and live more comfortably with your issue.

We understand that being diagnosed with diabetes can be a very upsetting and overwhelming experience. Our clinic provides comprehensive plans to help you more effectively manage your diabetes, using regular check-ups, ongoing education and selected activities to minimise the impact upon your lifestyle.
The diabetes clinic is run by our team of nurses, who work with your doctor and other health professionals, such as a podiatrist, ophthalmologist, pharmacist and diabetes educator, to ensure all facets of your health are equally considered.
Asthma Clinic
The key to living with asthma is keeping it under control. This means learning about what causes your symptoms, how to avoid the triggers and how to use your medications correctly for best effect. Our asthma clinic is run by our team of nurses, who work closely with your doctor to provide a comprehensive plan within a structured environment for further education and ongoing care.
