New Patients
At Langwarrin Medical Clinic we strive to provide the highest standard of medical health care that best fits the needs of our patients.
To ensure we appropriately cover all aspects of your medical history and existing conditions, patients new to the clinic are asked to make a double appointment. Please advise the Receptionist when calling to book, that you require a longer consultation.
New patients are requested to complete the Langwarrin Medical Clinic – New Patient Registration Form HotDoc.
Please bring any additional Concession Cards/DVA Cards to your appointment and advise the Receptionist on arrival and present your card. If a current Concession Card is not presented on day of consultation, we will not be able to process payment at Concession rate.
Please note that payment is to be made at the conclusion of your appointment. Payment can be made via Eftpos, Cash, Credit Card, Cheque and a Medicare Rebate claimed on the spot.
Telephone payments and Direct Deposit payments can also be made. Please note that if patient is not received at the time of consultation, an additional administrative fee may be applied to your account.
